Apparel Industry Certificate of Registration (LS-154)
This certificate is required if you make or alter any clothing or clothing accessories.Overview
All apparel manufacturers and contractors engaged in the making, cutting, sewing, finishing, assembling, pressing or otherwise producing of ANY means, women's, children's, or infants' apparel or section or component thereof MUST register. This includes but is not limited to apparel services industries such as contractors, jobbers, or manufacturers engaged in pleating, embroidery, stitching, buttons, belts, trimmings, shoulder pads and other accessories. Exemptions are the cleaning or tailoring after the apparel has been sold at retail.
Requirements
1. Photograph, Always Required2. Disability Insurance Certificate, Always Required
3. Copy of Certificate of Doing Business or Partnership Agreement or Certificate of Incorporation - whichever applies, Always Required
4. Worker's Compensation Insurance, Always Required
5. Bonding, Sometimes Required
6. Bonding - A surety bond not to exceed $2,500 per production employee is required as a condition of registration if found guilty of a second violation.
